Birth certificates are available for any child whose parents resided in the town of Plainville at the time of birth, whether the birth was in Plainville or another city/town within the Commonwealth of Massachusetts.
The fee to obtain a certificate is $10 per copy, payable at the Town Clerk's office by cash or check only.
To order via mail, please send a completed Birth Certificate Request Form (link below) to the Town Clerk's office, with payment and a self-addressed envelope enclosed.
For your convenience, online bill payment is now available for birth certificates. Go to online bill payment and select Plainville, MA to select the bill type for birth certificate. A convenience fee applies:
